AOSafety's Simple Solutions is a program designed to provide you with a cost-effective yet easy-to-administer safety prescription eyewear program. Our simple package pricing and minimal add-on options make it easy for you to understand what you are buying. Plus, our simple to use order form helps you outline the options you will allow your employees to order. Prices include frame and lens options, as well as professional fitting fees provided by one of our participating panel of Eye Care Professionals. Payment is easy too. Simple Solutions is a credit card only program. No need to establish an account or wait to order eyewear. Simply provide your credit card number at the time of order and we will promptly process your custom order for safety prescription eyewear.

Follow these four simple steps to order SRx eyewear from AOSafety:

• STEP 1: Download the order form.
  
  
• STEP 2: Print the form from your PC. Put the name of the patient the glasses are for - please print legibly -in the Patient Name section. Provide the patient's phone number in the space provided. On the right-hand side of the form you may choose to select the options allowed or not allowed in your program. If you leave options blank, the default choice will be followed by the Eye Care Professional and AOSafety when processing your order.
  
  
• STEP 3: Bring the order form to a participating Eye Care Professional (ECP). The ECP will assist you with the ordering process, including eyewear selection and vital technical measurements that we require to custom-make your safety prescription eyewear. A list of ECPs can be found on our website under Eye Care Professional Locator. If you cannot find a participating ECP within reasonable driving distance, please call 800.982.2828 and we will assist you. Orders must be dispensed by a current AOSafety participating ECP.
  
  
• STEP 4: Since orders cannot be processed without credit card information you have two alternatives to choose from:
  
  
  1. You may choose to provide your credit card information in advance by writing the credit card information in the space provided before you distribute the form to your employee(s). In this alternative, the ECP will fax the order directly to us.
     
     
  2. If you choose not to provide your credit card information in advance, simply request that your employee return to you with the completed form after they have visited the Eye Care Professional. Write the credit card information in the space provided and fax the order to us at 800.945.2828.
     
     

No credit card receipt will be provided, so please retain a copy of the order form for your records.
